Objective: The aim of this study was to evaluate the histopathological features of breast lesions detected in male patients, determine their diagnostic distribution, and assess the malignancy rate. Material and Methods: A total of 77 male breast lesion cases, evaluated by the corresponding author across different institutions and pathology laboratories between 2011 and 2025, were retrospectively reviewed. Demographic data and pathological diagnoses were re-evaluated and analyzed. Results: Patient ages ranged from 3 to 79 years, with a mean age of 39.2±20.8 years. Lesions were located in the right breast in 24 cases (31.2%), the left breast in 26 cases (33.8%), and were bilateral in 24 cases (31.2%). The lesion location was unspecified in three cases (3.9%). Malignant tumors were identified in 14 cases (18.2%). Among these, individual cases of diffuse large B-cell lymphoma, mucinous carcinoma, and cribriform carcinoma were diagnosed, while the remaining 11 cases were identified as invasive breast carcinoma of no special type (NST). In patients with malignant lesions, the age ranged from 58 to 79 years, with a mean age of 65.4±5.8 years. Among the 63 patients (81.8%) with benign lesions, the age ranged from 3 to 77 years, with a mean age of 33.4 ± 18.3 years. The difference in mean age between patients with malignant and benign lesions was statistically significant (p<0.001). Gynecomastia was the most frequent benign lesion, observed in 46 cases (73.0%). Conclusion: Although the majority of male breast lesions were benign, malignancy was detected in approximately one-fifth of cases. Malignant lesions occurred significantly more frequently in older patients, with invasive breast carcinoma representing the most common diagnosis. In the evaluation of male breast lesions, the possibility of malignancy must be considered, and rare malignancies such as lymphoma should be included in the differential diagnosis.
