EPS310(758 - Mixed Mullerian tumour with osteosarcomatous component in uterine corpus: report of an unusual case

INTRODUCTION
Malignant mixed mullerian tumor ( carcinosarcoma) is a rare neoplasm that derives from mullerian mesoderma and is differentiated in the epithelial and stromal element. Carcinosarcomas usually occur in elderly postmenopausal women, although occasional cases may occur in younger women and occasionally even in young girls prolapsed through the cervix.
CASE REPORT
A 62-year old woman complained of continuous vaginal bleeding of 2 months? duration. On magnetic resonance imaging, the uterine corpus was enlarged and an intrauterine polypoid mass was observed. After evaluation of the curettage material that was diagnosed as carcinosarcoma, total abdominal hysterectomy and bilateral salpingo-oophorectomy were performed. Histologically, the tumor consisted of mesenchymal and epithelial elements. The two components were distinct and demarcated but merging could be observed. The epithelial components consisted of complex and irregular glandular structures composed of cells with pleomorphic nuclei and frequent mitoses.The mesenchymal component was, for the most part, a high grade sarcoma, and heterologous elements including chondrosarcoma and osteosarcoma were also seen. These tumors have vascular invasion and deep myometrial invasion. The patient died of several organ and lymph node metastases, one month after the operation.
RESULT
In this report, we present the pathological features and diagnostic difficulties in malignant mixed mullerian tumor of the uterus. The reporting of this case is also of value, because malignant mixed mullerian tumors with associated osteosarcoma are rare.
